Acronym Corner: The Aftercare ACL 25-27 / BHN 25-027 is Here!

Decoding Acronyms and Expectations Together

The long-anticipated (and yes, even slightly feared) Aftercare guidance is finally here! While the expectation to provide six months of Aftercare services via Wraparound has been in place since 2021, the new ACL 25-47/BHIN 25-027 introduces updated expectations. It clarifies that any Wraparound provider contracted for Aftercare services must now be aligned with the newly revised Wraparound Standards and must demonstrate that alignment through the formal certification process.

But do not panic! Counties and providers have a full year from the release of the ACL/BHIN date to initiate and complete their plan approval (counties) or program certification (providers) through the Portal (online at https://calwrapcertification.com), and the recently launched California High Fidelity Wraparound Technical Assistance (TA) System offers a wide array of resources. 

And Here Come the New Acronyms

Naturally, with new guidance comes a whole new batch of acronyms; so, let’s decode them together!

In case you are wondering or your spell check is going crazy wanting to add a hyphen between “high” and “fidelity,” leave it out! No hyphen needed for the California High Fidelity Wraparound Model or related titles.

Reminder: Being in-the-know of these new short-hand terms will help your families feel current, comfortable and confident in conversations when you share  and help decode these with them.

California High Fidelity Wraparound Model (CA HFW Model)

California's version of high fidelity Wraparound, based on the California Wraparound Standards. This model is now the required standard for Aftercare services. It’s a team-based, evidence-informed approach with an “anything necessary” mindset for youth and families facing the most intensive challenges.

High Fidelity Wraparound (HFW)

A broader term referring to the overall approach, practices, programs and providers delivering high fidelity Wraparound services.

High Fidelity Wraparound Team (HFW Team)

The dedicated group of HFW staff who become part of the Child and Family Team (CFT). They work with system partners like education, Tribes, regional centers and natural supports to co-create a strengths-based plan of care that aligns with family goals and required services.

Wraparound Team Meeting (aka: Family Team Meeting) (HFW Team Meeting)

When a youth and family are receiving HFW, the HFW staff become part of the CFT meetings. The HFW Team meets more often than the CFT requirements to plan and implement the HFW process. Due to the frequency of HFW team meetings, not all CFT members are required to attend every HFW Team meeting. However, the HFW team ensures all CFT members stay informed and connected to the discussions within the HFW team meetings.
