4 Phases

1. Engagement and Team Preparation 

  • Creates initial contact between the HFW team, youth and family 
  •  Introduces the HFW process
  • Identifies CFT members (if one does not already exist)
  • Addresses immediate needs
  • Discusses youth and family’s needs and strengths

2. Plan Development

  • Develops individualized care plan reflecting the child or youth and family's needs, strengths, and strategies to build shared vision among the CFT.

3. Implementation

  • Implements individualized care plan
  • Reviews and updates individualized care, crisis and safety, and transition plans
  • Begins shift of responsibility to family and natural supports

4. Transition

  •  Prepares youth and family for transitioning out of HFW
  • Continues to shift responsibility to family and natural supports
  • Organizes closing ceremony at the end of HFW
  • Celebrates success to facilitate a positive transition
