This skills-based class will provide an opportunity to practice case planning with transition-aged youth who are entering extended foster care. With a focus on keeping all efforts centered on the youth experience throughout the process, trainers will share and offer participants an opportunity to practice with specific tools such as Supportive Transition Team Meetings, Transitional Independent Living Plans (TILPs), 90 Day Plans and Permanency Pacts.
After attending this training, participants will be able to:
- Understand key concepts relevant to transition aged and nonminor dependent youth, including court and other legal considerations, eligibility, case plans, case management, documentation, transition planning and placement considerations
- Identify and utilize tangible tools to use for effective case management of transition aged youth and nonminor dependents
- Given a case example or scenario, conceptualize legal, court, and case management information and social worker responsibilities from a youth-centered perspective
- Apply effective strategies to minimize re-traumatization and attachment disruption
- Articulate the value of putting the youth experience at the center of their work with older youth populations
