Transition Planning in Wraparound Skills Lab

This training is a follow-up course to the Foundational Wraparound training and the Wraparound Plan of Care course. Participants will increase their skills in the process of identifying progress in Wraparound and creating appropriate plans for transition from formal systems. This course is structured as a hands-on skills lab with minimal traditional instruction, encouraging participants to actively practice and refine their techniques for evaluating the plan of care, safety and crisis plans, and the creation of sustainable transition plans.

The focus will be on skill development in the following areas:

  • Evaluate a sample plan of care based on high-fidelity Wraparound practices to determine if the plan meets the goal of independence and sustainability for the youth and family.
  • Write a transition plan based on elements and structure provided for a case scenario.
  • Write a post-transition crisis plan for a case.
  • Develop ideas on how to celebrate successes of a wraparound team.
  • Prioritize diversity, equity and inclusion in the creation of plans and services provided to youth and families

Join us to enhance your skills and learn effective strategies for supporting families through the Wraparound transition process, ensuring sustainable, individualized, and culturally relevant support!
