Transition Planning in Wraparound Skills Lab
This training is a follow-up course to the Foundational Wraparound training and the Wraparound Plan of Care course. Participants will increase their skills in the process of identifying progress in Wraparound and creating appropriate plans for transition from formal systems. This course is structured as a hands-on skills lab with minimal traditional instruction, encouraging participants to actively practice and refine their techniques for evaluating the plan of care, safety and crisis plans, and the creation of sustainable transition plans.
The focus will be on skill development in the following areas:
- Evaluate a sample plan of care based on high-fidelity Wraparound practices to determine if the plan meets the goal of independence and sustainability for the youth and family.
- Write a transition plan based on elements and structure provided for a case scenario.
- Write a post-transition crisis plan for a case.
- Develop ideas on how to celebrate successes of a wraparound team.
- Prioritize diversity, equity and inclusion in the creation of plans and services provided to youth and families

Who should attend?
This course is appropriate for all members of the Wraparound team, including facilitators, (care-coordinators), peer partners, family specialists, supervisors, and others.
Course Pre-Requisite Recommendations:
This course is a Skills Lab that requires active participation in a hands-on learning environment. To participate, it is recommended that learners:
- Have attended the Foundational Wraparound course.
- Completed the Wraparound Plan of Care course.
- If possible, attended the Wraparound Safety and Crisis Planning Skills Lab courses.
- Highly recommended, has attended or been involved in Wraparound meetings.
- Be present for the entire duration of the course (leaving early/coming late is disruptive to the designated learning environment).
- Be willing and able to be on camera and engage in collaborative learning with peers.
