Effective APS Case Documentation: Clarity, Accuracy, Consistency and Accountability

 Course Overview

This engaging, interactive training strengthens Adult Protective Services social workers’ ability to produce ethical, consistent and defensible documentation within the LEAPS system. Designed for journey-level practitioners, the course presents documentation as a core social work practice skill that supports sound professional judgment, equity, client safety and accountability.

Grounded in the NASW Code of Ethics, participants apply APS documentation standards through structured casework, guided review and editing exercises aligned with supervisory expectations. The course introduces a defensible documentation spine that helps participants clearly align allegations, evidence, analysis and findings without prescribing a single narrative format.

Learning Objectives

After attending this training, participants will be able to:

  • Identify required APS documentation elements in LEAPS that support ethical, defensible decision-making across the life of a case.
  • Differentiate among factual observation, professional analysis and findings determinations in accordance with social work documentation standards.
  • Apply APS documentation standards to create accurate, timely and defensible LEAPS case entries.
  • Construct APS narratives that logically and ethically align allegations, evidence, analysis and findings.
  • Review and revise APS documentation using a standardized case review checklist consistent with supervisory and quality assurance expectations.
  • Demonstrate professional judgment by clearly documenting the rationale for APS decisions in a manner that withstands supervisory, audit and external review.
  • Exhibit accountability for consistent, unbiased and equitable documentation practices that promote client safety, regulatory compliance and adherence to professional social work ethics.

Fee Details: The enrollment fee for this 1-day class is $255.00 per participant or .06 training units for those utilizing their county contract. This fee includes quality instruction from subject-matter expert(s), research-based curricula and all digital course materials.

Training Format: This program will be offered via remote learning using Zoom. Login information will be provided after your enrollment is processed.
