Plan of Care Skills Lab

The Plan of Care Skills Lab is intended to be a follow-up course to the Foundational Wraparound training to take a deeper dive into developing Wraparound Plans of Care with children, youths, and families. This course is designed as a skills lab with minimal traditional instruction, where participants will take an active role in practicing the Wraparound planning process while developing plan of care.

This course is appropriate for all members of the Wraparound team, including facilitators, peer partners, family specialists, clinicians, supervisors and other system partners who participate on Wraparound teams. It will focus on skill development in the following areas:

  • Practice identifying underlying needs from unwanted behaviors/presenting concerns
  • Brainstorm strategies that address underlying needs
  • Identify outcomes that are needs-driven and develop corresponding SMART goals
  • Assign strategies based on strengths using a sample team strengths document
  • Collaborate with co-learners to develop a Wraparound Plan of Care using the best practice standards.

Course Requirements:

This course is a Skills Lab that requires active participation in a hands-on learning environment. To participate, learners must:
